If you have claimed the Earned Income Credit, and the IRS asks you to
prove all the qualifications have been met, then you should expect the
IRS to ask you for the following documents.
For each dependent claimed on the tax return the IRS will ask you to
furnish:

Copy of taxpayer's social security

Copy of spouse's social security card

If the school records are not
available or there are none, then you may substitute:

A statement on company
letterhead or a notarized statement from a child care
provider containing the child's name, address, and
dates of care fro the entire tax year, the name and
address of the child's parent or guardian, and the
name and taxpayer identification number of the child
care provider, or |
 |
Medical records or an
administrative statement from a health care provider
containing the child's name, address, and dates of
medical care for the tax year, and the name and
address of the child's parent or guardian. |

If you received any government
benefits obtain a statement from the government agency verifying
the amount and type of benefits |
 |
If the dependent is more than 18 and
disabled obtain an official letter or document stating the
dependent is disabled |
 |
Copy of Postal Form 1093 if the
address on the return was a post office box |

If the child's mother and father is
not that of the taxpayer or spouse, then furnish the following for
each child's mother and father

agreement or your last mortgage statement for the year |

expenses paid during the tax year. |

Non-custodial Parents and Guardians:
 |
Copies of canceled checks
verifying support payments or a statement of account from
your child support agency. |

Divorced and Legally Separated Parents:
 |
Copies of your divorce decree or
separation agreement. Include all pages/revisions. |
 |
Copy of any other written
agreement showing which parent has custody. |

Copy of form 8332, release of
claim for dependent, signed by the other parent, if
applicable.
