*IMPORTANT:  For an individual, annual contributions may be made up to 65 percent of the high-deductibility policy's annual deductible amount.  For family coverage, the maximum contribution is 75 percent.

For tax year 2003 a high deductible plan is defined as one having an annual deductible amount of $1,700 to $2,500 for an individual, and $3,350 to $5,050 for a family, and having an out-of-pocket expense maximum of $3,350 for individual coverage, and $6,150 for family coverage.
